If every chart you create looks basically the same, it might be a problem for CMS.

 

Sure, EHR templates save time and make charts more complete. But if you rely on them too heavily, your chart can also be inaccurate. If your billing and charting makes it seem like every patient you see is the same and gets the same treatment, your payers might start to wonder how much of that work you actually are doing.
